Questions Regarding the Creation of Identity Among the Inker and the Inked:

In the world of tattooing, who is doing the writing?  Clearly, the artist does the physical writing on the bodies of those being inked, but who creates the visual text?  Is there a different feeling of creativity between being told what to ink versus being given an idea and the freedom to create the text based on that idea?  How does a tattooist create identity for her/himself as well as her/his subject through this creativity?  How do the identities created differ on the spectrum of creativity the tattoist is allowed?
